MomSub Child Care Provider Background Check Form

We're so glad you've joined our community of caregivers!

We want everyone to feel comfortable that all our providers are safe, so we run a background check on every MomSub. We need to complete this check before we can schedule you for an interview.

​

As with all our MomSubs, we require you to fill out the information below and submit it through this secure form to complete your application for a position as a child care gig worker.

​

The one-time cost for the background check is $19.95, and you will be receiving an invoice for this charge. We do not charge child care providers for access to our platform, but we have found that nannies who pay for their background check are more likely to take the job offer seriously and show up for their in-person interview and their scheduled work shifts.
